For your convenience we have provided the forms we’ll need at the time of enrollment.

  5. Pediatric Health Statement (for infants through Pre-K only)

Enrollment Requirements and
Health Care Records

The Registration Enrollment forms and contact forms must be completely filled out and signed by the parent or guardian. This enrollment pack needs to be returned BEFORE the FIRST DAY of your child’s participation in our program. Pre-kindergarten students and younger must have an Immunization record. Records are kept on each child and must be updated regularly. Please let us know if any changes occur and said changes will be noted on the records.


Our Licensing Board requires complete immunization records for each child enrolled.  A statement signed by the doctor must confirm this. Pre-school (4 and 5 year olds) children are required by the State Health Department to have a Hearing and Vision Screening Test on file at the center.
